John Mitchell from CR Automation presents the background and consequences of a modification to a safety-related control system for a nuclear shield door.

Several failures in the management of change control procedure could have resulted in an accident with fatal consequences.

John worked in the nuclear reprocessing industry from 1983 to 2005 where he started as apprentice electrician and finished as senior control systems project engineer. John was also part of a small team that implemented IEC 61508 and standardised permit to work systems to the Sellafield site.
